Your Orientation
Your orientation to Wheaton Franciscan Healthcare will consist of self-study materials completed prior to your arrival and a department/facility orientation on your first day of work.
Step One: Credentialing Checklist
Before working at a Wheaton Franciscan Healthcare facility, a Credentialing Checklist must be completed. Please work with your sponsoring physician and Medical Staff Office to complete/provide this information.
Step Two: Forms
The following forms must be printed, completed, signed and brought with you the first day of work in order to document your orientation.
Allied Health Orientation Checklist
Please print a copy of the Allied Health Orientation Checklist. Review all items listed under Orientation Materials and check them off on the list. You must complete both the top and bottom portions of this check list. If any of the items listed under the Clinical portion are not applicable to your role, please mark with NA. This list must be signed, dated, and turned in to your Medical Staff Office. At the time of your facility orientation, the site-based orientation items will be completed and signed off.
Confidentiality Statement
Please print, review, and sign the Confidentiality Statement and turn it in to your Medical Staff Office.
The Journey
Please review the Expections for the Journey .
Please print, review and sign the Expectations for the Journey Commitment Statement. Bring it along on your first day of work to turn in to the manager or supervisor of the department you will be working.
Step Three: Self-Study
The documents listed under Orientation Materials cover key information that you need to know before your first day at a Wheaton Franciscan facility. Our goals are that you will feel comfortable and knowledgeable in the environment and that you will be able to meet the same high standards of behavior and patient care that are expected of our own associates. The topics correspond to items listed on the Orientation Checklist. Please review each document carefully before checking it off on the Orientation Checklist.
Step Four: On-Site Department/Facility Orientation
The Medical Staff Office will arrange for your on-site orientation. Bring the completed, signed, and dated Orientation Checklist, Confidentiality Statement and Expectations for the Journey Commitment Statement with you.
