Diabetes, one of the most common chronic conditions in the world, occurs when your body fails to process sugar properly causing blood sugar levels to become too high. It is a lifelong condition, but you can control it and take charge of your life.

Take Back Your Life
The Diabetes Self-Management Education Program at Wheaton Franciscan Healthcare – All Saints in Racine has been awarded Recognition by the American Diabetes Association in accordance with the National Standards for Diabetes Self-Management Education Program.

Interdisciplinary Approach Through this program, we will teach you how to control your disease and lead an active life. We use an interdisciplinary team approach including one-on-one counseling and support groups to educate anyone who has diabetes, including children and their parents, pregnant women and adults.

Appointment Scheduling
Your doctor or program staff may schedule an appointment or you may schedule one yourself. Be sure to have the following on hand for your first appointment:

All medications, including vitamins and herbal supplements
A two-day record of all food eaten
If you’re on insulin, bring your insulin and syringes
Your blood sugar meter and blood sugar record, if you test, along with other diabetes supplies

Program Cost
The cost of the program varies. It may be covered by health insurance or Medicare/Medicaid.
