Step 4: Computer Password Access Request
All ADU/Diebold/ Computer access requests go through the academic affiliations project coordinator, not the IS department. This process begins by completing the clinical confirmation form under the “on-line forms” tab on our web site. Please submit all student names (precepted/clinical student groups) at least two weeks prior to the beginning of the semester, this will allow processing of your request to begin. For more information, you may contact Sarah Martinez (Milwaukee) 414-465-3443 or Terri Buckett (Racine) 262-687-4852.
Please do not fax or call the IS department with access requests as it actually slows the process down. If IS receives any of these requests, they will be re-directed to the project coordinator for approval, as paperwork (health records, criminal background checks, etc.) must be verified prior to giving any student/instructor access of any kind.
